Dr. Sijia Wei is a Postdoctoral Fellow at the Center for Education in Health Sciences within Northwestern University Feinberg School of Medicine. She earned her Ph.D. in Nursing Science from Duke University School of Nursing and is a registered nurse and public health nurse with clinical experience in community-based health care services (i.e., long-term care, home health care, rehabilitation, and hospice). Her research focuses on care coordination in care transitions across settings and over time by evaluating relational processes and social mechanisms essential to care coordination using social network analysis and large datasets such as electronic health records. She seeks to understand the complex systemic factors that contribute to health disparity, with the ultimate goal of enhancing the quality of care and promoting health equity for people with chronic care needs in the community.
