A. Personal Statement
:For the past 15 years, I have been the Principal Investigator at Gaylord Hospital for the Spaulding New England Regional Spinal Cord Injury Model, and most recently as Co-Director Clinical Care. I am board certified in not only Physical Medicine and Rehabilitation, but additionally in Spinal Cord Injury Medicine and Brain Injury Medicine. I have demonstrated the expertise and enthusiasm necessary for fulfilling Gaylord’s role as part of the Model System. My previous extensive background in spinal cord injury treatment and rehabilitation has led me to work on various topics related to promote the health of people with disabilities and prevention of secondary conditions. I led the initiative on NESCIT, the New England Spinal Cord Injury Toolkit, which was developed collaboratively with many other centers and clinicians in New England, as part of our Model SCI work.
My previous extensive background in spinal cord injury treatment and rehabilitation has led me to work on various topics related to promote the health of people with disabilities and prevention of secondary conditions. At Gaylord, I serve as the Medical Director of the Milne Institute for Health Care Innovation, Director of our Spinal Cord Injury Program and as the Research/Site Director of the Model Systems SCI Program. I have been very effective in my role in current grant cycle to ensure success with the SCIMS national database through clinical coordination and training, and I have provided oversight for Gaylord for multiple phases of SCI studies, notably site-specific SCI-FI iterations and My Care My Call, and our multi-site collaborations (SCI-FI, SCI-QoL, PROMIS, and EQuATe).
Based on my previous experiences, I understand the importance of frequent and relevant communication amongst all project members, the need for a sound and realistic research plan including budgets both monetary and time, and a set timeframe for completion. This application builds upon my successful past research and it will enable a greater population to benefit from our findings.

C. Contributions to Science
For the past three SCI Model System grant cycles, I have led the NIDILRR funded Model System work at Gaylord Hospital. I have taken an active role in the development of the research base at Gaylord and the surrounding areas. When appropriate community members and professionals throughout the area have been included in our SCI research and education opportunities.
Notably, I led the development of the New England SCI Toolkit. Designed to provide a standard of care with corresponding resources, NESCIT provides users, which include professionals and people new to SCI care with an easy to use resource to learn about common SCI care concerns.

Prominent in my work is the development of the Spinal Cord Injury Functional Index and its multiple clinical iterations which allows for patient reported outcomes in physical tasks across the continuum of care from acute rehabilitation to chronic care appointments. Developed specifically for people with an SCI, the SCI-FI scales are sensitive enough to captured minute changes in a person’s function. This includes developing Short Forms for certain SCI-FI item banks to be used when technical challenges are presented as well as different age appropriate assessments to be used from childhood through adulthood.

Research that includes people with an SCI as active participants in learning to manage their own health has led to the development of the SCI Peer Health Coach (PHC) role. In the My Care My Call study, an SCI PHC met with participants by telephone and text to provide health-coaching services about topics related to SCI. The PHCs took on the roles of Role Model, Supporter, and Advisor during coaching sessions that were based on topics the participants chose to discuss. Primary outcomes of this intervention showed the peer-led, health self-management intervention achieved a positive impact on self-management to prevent secondary conditions in adults with SCI.

My clinical work lends itself to presenting topics of relevance to build upon and improve the care of individuals with SCI and MS and some of the comorbidities that may develop from them. Important in my work is the relevance of understanding the patient’s current experience and using that to inform their care.

Furthering the capacity building of future generations of SCI clinical professionals, I am involved in various aspects of education regarding traumatic and rehabilitation care. In addition to my role as the Director of Medical Education at Gaylord, I coordinate the Yale Neurology rotation and an Associate Clinical Professor at Yale University School of Medicine in the Department of Neurology and Orthopaedics & Rehabilitation.
