Development and Implementation of an Early Detection Program: Pitfalls and Successes

Research Objectives: Cerebral palsy (CP) is the most common disability in childhood, but often goes undiagnosed until age 2 years. Watchful waiting and monitoring have historically been practiced. International guidelines set forth in 2017 led to a paradigm shift towards early detection and intervention. Our institution, an affiliate of United Cerebral Palsy (UCP) with support from the Cerebral palsy foundation (CPF) has established an early detection program. Our objective is to share our program development process. ​

Design: A chart of ‘strengths, weaknesses, opportunities, and threats (SWOT) was first created, and key drivers identified. Opportunities to convert ‘weaknesses’ into ‘strengths’ were looked at and worked on. The group met monthly and refined the process.

Setting: Large pediatric rehabilitation hospital

Participants: Executive directors, program managers, physicians, and therapists participated​

Interventions: Educate all stake holders. Trained therapists on appropriate evaluations. Developed protocols for Evaluations, early detection and interventions of infants and toddlers.

Main Outcome Measures: 1. Increase in referrals of infants and toddlers under 2
2. Diagnosis given before 12 months of age

Results: We have successfully developed an early detection protocol and are launched the new program within our institution. Therapist training for infant assessments has been completed. Information regarding this initiative is being shared within our institution and with our community partners to increase early referrals.

Conclusions: Early detection and intervention of CP is best practice. 
We have successfully instituted an early detection program at our hospital. We showcase our process and share our successes and struggles to provide a framework for other centers looking to establish a new program
