Reliability and Validity of the Japanese Version of the Independently Get Off the floor Scoring System in Stroke Survivors

Research Objectives: To investigate inter-rater and intra-rater reliability and construct validity of the Japanese version of the Independently Get Off the floor scoring system (IGO-J) in chronic stroke survivors.

Design: Cross-sectional study. Kappa coefficients and Spearman correlations were used to analyze data.

Setting: Community.

Participants: Fifty-one chronic stroke survivors who can walk independently, using day-care rehabilitation. The protocols were approved by the Japan health care University Ethics Committee (code: 2023-11).

Interventions: Not applicable.

Main Outcome Measures: An English version of the IGO has been already created, that can measure the capacity to stand up from the floor, but the Japanese version of that has not been validated. We created the IGO-J using the back-translation method. To verify the inter-rater reliability, two examiners independently scored. To verify the intra-rater reliability, one examiner scored the IGO-J twice with an interval of two months. Video of the participant’s movement that standing up from the floor was used to score the IGO-J. To verify the construct validity, we also evaluated the modified Rankin Scale (mRS), Stroke Impairment Assessment Set (SIAS), lower limb muscle strength, 5 repetition sit-to-stand test (5-sts), Timed Up and Go test (TUG), Stroke Impact Scale-16 (SIS-16), and Falls Efficacy Scale-International (FES-I) were examined, respectively.

Results: For the inter-rater and intra-rater reliability, the Kappa coefficients was 0.817 and 0.725 (p < 0.0001), respectively. The IGO-J also found to be significantly correlated with the TUG (rs=-0.521, p=0.0001), SIS-16 (rs=0.665, p< 0.0001), mRS (rs=-0.411, p=0.003), SIAS-shoulder ROM (rs=0.493, p=0.0002), and SIAS-trunk (rs=0.355, p=0.011). No significant correlations were found between the IGO-J and lower limb muscle strength, 5-sts and FES-I.

Conclusions: The IGO-J is a reliable measurement tool that correlates with functional mobility, perceived physical disability, activity of daily living, and stroke-derived functional impairments which especially in the shoulder and trunk functions but not lower limb muscle strength and falls self-efficacy. The IGO-J will be useful in rehabilitation for improving floor-living life style.
